Our homes benefit seniors who may not require nursing home care but are no longer willing or able to care for themselves in their own home and yet want to maintain an independent lifestyle. We will provide individual assistance with mobility, personal care, prompting throughout the day guidance, general activities and dispensing of medications. Our home will be designed to meet the needs of clients who have a diagnosis of Alzheimer’s/mild dementia who require Level 2 & 3G Care. These clients will require assistance with all Activities of Daily Living (ADL and IADL). Resident care will be planned, coordinated, provided, delegated, and supervised by professional health care providers who access and recognize the unique physical, emotional, and spiritual needs of each person. Professional medical care will be provided through office visits, clinics, home visits, Extra mural, Community Liaison Nurses.
